Indications/Uses
Treatment of serious anaerobic & some Staph & Strep infections; prophylaxis of endocarditis & surgical infection.
Dosage/Direction for Use
Adult Serious infections 150-300 mg 6 hrly. More severe infections 300-450 mg 6 hrly. Acne 150 mg 12 hrly. Bacterial vaginosis 300 mg 12 hrly for 7 days. Cervicitis due to Chlamydia trachomatis 450 mg 6 hrly for 10-14 days. Pelvic inflammatory disease (inpatient treatment) Initially clindamycin IV 900 mg 8 hrly + gm -ve aerobic antibiotic IV spectrum for at least 4 days & 48 hr after patient improves then continue w/ oral clindamycin HCl 450 mg 6 hrly for 10-14 days. Prevention of bacterial endocarditis in penicillin-allergic patients 600 mg as a single dose 1 hr prior to the procedure. Pneumocystis jirovecii (carinii) pneumonia in AIDS patients 300-450 mg 6 hrly + primaquine for 21 days. Alternative treatment of multidrug resistant Plasmodium falciparum infection 20 mg/kg/day divided in 3 equal doses + quinine or amodiaquine for at least 5 days.
Administration
May be taken with or without food: Take w/ full glass of water to avoid esophageal irritation.
Contraindications
Hypersensitivity to clindamycin or lincomycin. Development of pseudomembranous colitis. Newborn (infants <30 days).
Special Precautions
Not for the treatment of nonbacterial infections & meningitis. Clostridium difficile-associated diarrhea (CDAD). Atopic individuals as well as history of GI disease particularly colitis. Concurrent use w/ neuromuscular blocking agents. G6PD deficiency. Perform blood exam during therapy w/ primaquine & surgical drainage & necrotic tissue removal in combination w/ antibiotic therapy. Severe renal &/or hepatic impairment accompanied by severe metabolic aberrations. Monitor serum levels for high-dose therapy. Periodic blood counts, liver & kidney function tests during prolonged therapy. Increased risk of antibiotic resistance. Overgrowth of nonsusceptible organisms including fungi; liver function tests abnormalities. Pregnancy & lactation. Childn ≤16 yr. Elderly >60 yr.
Adverse Reactions
CDAD & colitis/pseudomembranous colitis frequently accompanied by fever & leukocytosis. Generalized mild to moderate morbilliform-like & vesiculobullous rashes, urticaria, pruritus, fever, hypotension, anaphylactoid reactions. Jaundice, hepatic damage, transient increases in serum bilirubin, alkaline phosphatase, ALT, AST, BUN. Leukopenia, eosinophilia, thrombocytopenia, agranulocytosis, leukocytoclastic angiitis.
Drug Interactions
Neuromuscular blocking agents, aminoglycosides, ampicillin, erythromycin.
